Social Worker - MSW

Aspirus At Home is seeking a Social Worker - MSW to join our Hospice field team covering sites within Northern Wisconsin.

The Hospice Social Worker, under general supervision and in accordance with established policies and procedures, assists patients and families in coping with end‑of‑life concerns. This includes obtaining psychosocial histories, resolving environmental obstacles to quality of life, conducting discharge planning, and directing patients to community resources as needed. The Social Worker is a core interdisciplinary member providing essential social work services under the direction of a physician.

Hours:

Full time or 1.0 FTE, 80 hours every pay period, 8‑hour day shifts Monday – Friday.

Experience / Qualifications
  • Knowledge of social work theories, therapies, casework processes, and public welfare principles acquired through a master’s degree in Social Work from an accredited program.
  • Coursework in Medical Social Work beneficial.
  • Academic concentration on psychosocial aspects of terminal illness and bereavement, and education in death, dying, grief, and loss beneficial.
  • Experience working with the terminally ill and on multidisciplinary teams preferred.
  • Previous professional experience with Hospice, medical, and home settings beneficial.
  • Minimum of one‑year medical social work experience required.
  • Wisconsin or Michigan Certified Social Worker licensure required in the state of practice.
  • Must drive a motorized vehicle throughout the service area on a regular basis.
  • Valid Wisconsin or Michigan Driver’s License required.
  • Automobile liability insurance with minimum $100,000/$300,000 coverage required.
  • Excellent interpersonal skills to obtain information from patients and families, provide counseling, and interact effectively with patients, families and hospital personnel.
  • Strong analytical skills to assess patients’ needs, develop discharge planning, and provide sound advice.
  • Ability to concentrate and pay close attention to detail during interviews, therapy, and discharge planning.
  • Ability to cope with a work environment that deals with death, dying, and bereavement.
  • Tolerance for exposure to animals.
