Travel Nurse - Registered Nurse - Home Health

Role

Home Health Registered Nurse (RN) responsible for providing direct patient care in home settings.

• Assessing patient conditions and developing individualized care plans based on medical needs and physician directives.
• Administering medications, treatments, and wound care while monitoring responses.
• Educating patients and caregivers on disease management, medication use, and self-care techniques.
• Monitoring patient status, reporting concerns, and adjusting care plans accordingly.
• Coordinating with healthcare professionals to ensure seamless care transitions.
• Documenting care, interventions, and progress accurately and complying with regulatory standards.
• Supporting families and caregivers with guidance and emotional support.
• Maintaining communication with interdisciplinary teams and completing required charting.

• Minimum one year of recent RN experience, with at least six months in home health.
• Proficiency in OASIS documentation and valid RN license.
• BLS certification required; additional certifications like RN-BC preferred.
• Critical thinking, strong communication, and interpersonal skills.
• Physical ability to perform patient care tasks and move equipment.
• Ability to work effectively under pressure, follow instructions, and maintain professionalism.
• Visual acuity and effective communication skills.
