Palliative Nurse Practitioner

Palliative Nurse Practitioner We are seeking a Nurse Practitioner to join our growing palliative care program. This is an opportunity to make a meaningful impact by supporting patients with complex, chronic, or serious illnesses in a collaborative, patient-centered environment.

Position Details:

This position is PRN with the possibility of growing into part time or full time Responsibilities:

Conduct comprehensive palliative care visits, including assessment, diagnosis, and management of symptoms

Complete F2F encounters for hospice patients as needed

Develop and implement individualized care plans in collaboration with the interdisciplinary team Prescribe medications and coordinate care for patients with advanced or chronic illnesses

Accurately document all encounters in the EMRCommunicate effectively with patients, families, and other healthcare professionals

Requirements:

Experience preferred but willing to train new Nurse Practitioners!

Active Nurse Practitioner license in Wisconsin Strong clinical assessment, documentation, and care planning skills preferred

Experience with eClinical

Works (eCW) EMR preferred (willing to train)
Excellent communication and teamwork abilities
