Hope Health is seeking an experienced Patient Care Manager to lead the interdisciplinary team delivering hospice and palliative care in Rhode Island South and Newport areas. Based in Wakefield, RI, this full-time role oversees clinical services and ensures 24/7 patient care coordination by the IDG.
Requirements include RN with BSN, RI/MA licensing, and at least 3–5 years nursing experience with home care/hospice. Supervisory experience and HPNA certification within 1 year are preferred.
