Position Overview:

This full-time role is open to Nurse Practitioners (FNP, AGACNP, ACNP, AGPCNP) and Physician Assistants passionate about post-acute care. You???ll work with an interdisciplinary team, ensuring smooth care transitions while collaborating closely with hospice, palliative, and home care agencies. Responsibilities:
Deliver high-quality, cost-effective patient care in compliance with CMS guidelines Collaborate with clinicians and care teams to optimize patient outcomes Complete timely documentation and billing through EMR Maintain positive relationships with patients, families, and staff Compensation & Benefits:
