PRN Home Health Physical Therapist (PT)

We are seeking a compassionate Physical Therapist to provide personalized rehabilitative care to patients in their homes. You will help patients improve mobility, relieve pain, regain strength, and achieve greater independence.

• Perform comprehensive initial evaluations and identify patients' functional needs.
• Develop and implement individualized treatment plans.
• Provide therapeutic interventions to improve mobility, strength, balance, and overall function.
• Evaluate patient progress and adjust treatments to achieve the best possible outcomes.
• Educate patients and caregivers on exercises, safety, and ongoing care.
• Communicate effectively with patients, families, physicians, and interdisciplinary team members.
• Follow infection-control and universal-precaution standards.
• Complete accurate clinical documentation within company guidelines.
• Promote continuity of care and uphold agency policies and quality standards.

• Graduate of an accredited physical therapy program.
• Current and valid Physical Therapist license in the state of practice.
• CPR certification.
• Valid driver's license and automobile insurance.
• At least one year of home health experience preferred.
• Home Care Home Base experience preferred.
• Strong communication, organization, and time-management skills.

• Ability to stand, walk, sit, climb, crawl, squat, or kneel for extended periods.
• Ability to lift, carry, push, or pull up to 100 pounds with appropriate assistance or equipment.
