This is a non-bedside role for a Licensed Practical Nurse (LPN) to support patients enrolled in Medicare’s Remote Patient Monitoring (RPM) and Chronic Care Management (CCM) programs.
** Your Contribution:
*** Create, update, and review patient care plans based on the provider’s plan of care
* Conduct monthly telephonic check-ins with patients to assess health status and medication adherence
* Review RPM synced vital signs and escalate concerns to the provider when appropriate
* Coordinate follow-ups, scheduling, and documentation updates
* Ensure all documentation meets Medicare CCM and RPM compliance guidelines
* Accurately document all patient interactions within multiple EHR systems

Qualification:

*** Active LPN License (Multistate Nurse Compact License Required)
* Strong verbal and written communication skills
* Minimum 1 year in related Chronic Care Management services
* Experience working with EHR systems
* Ability to work independently with minimal supervision
* Reliable high-speed internet and a quiet, HIPAA-compliant workspace
