Job Summary

This role assists patients, families, and caregivers in overcoming healthcare barriers, including social, financial, or physical challenges. The Oncology Nurse Navigator conducts ongoing assessments of needs, facilitates referrals to support services throughout the oncology continuum, and provides education and resources to enable informed decision‑making.

Education and Experience
  • Education
    :
    Associate's Degree Graduate of an approved/accredited nursing program either by a governmental authority, nursing board, or by a licensing authority that has jurisdiction over approval/accreditation of nursing programs (Texas BON 217.1)
  • Experience
    : 3 years oncology nursing experience
  • Licenses and Certifications
    :
    Registered Nurse (RN) and Basic Life Support (BLS) course accredited by the American Heart Association (AHA) or American Red Cross (ARC)
Preferred Qualifications
  • Education
    :
    Bachelor of Science in Nursing
  • Licenses and Certifications
    :
    Oncology Certified Nurse (OCN), Certified Pediatric Hematology Oncology Nurse (CPHON), Certified Breast Care Nurse (CBCN), or Blood Marrow Transplant Certified RN (BMTCN)
Job Duties
  • Assesses for barriers to care and addresses needs of patients, caregivers, and families throughout the cancer care continuum.
  • Serves as a point of contact for providers, patients, and caregivers.
  • Advocates for patients and caregivers navigating the healthcare system.
  • Facilitates education for patients and staff through appropriate programs and resources.
  • Provides education, resources, and referrals to internal and external resources based on assessment needs.
  • Promotes shared decision making, ensuring informed treatment intent, options, and outcomes.
  • Encourages patients to convey wishes and goals of care and formulates questions for providers.
  • Facilitates clear and concise communication among providers, departments, and patients.
  • Coordinates multidisciplinary planning and facilitates care conferences as necessary.
  • Participates in outreach projects to close patient care gaps.
  • Performs other duties as assigned.
