New Century Hospice is seeking an After hours RN Care Manager to provide high-quality hospice care outside standard hours, ensuring seamless coverage and strong clinical management for patients and families during nights, weekends, and on-call hours.
The RN will assess and coordinate patient and family needs during after-hours shifts, maintain primary clinical oversight, and participate in the Interdisciplinary Group to ensure coordination between physicians, families, and the hospice team.
