Sales Specialist

Ayerdi is seeking a compassionate and detail-oriented Case Manager to support clients by assessing needs, coordinating services, and advocating for resources. This role requires strong problem-solving and organizational skills to ensure individuals receive comprehensive support tailored to their circumstances.

Key Responsibilities:

Conduct client assessments to determine needs and develop individualized care plans.

Coordinate with service providers, healthcare professionals, and community organizations.

Advocate for clients to access essential services, including housing, healthcare, and employment support.

Maintain detailed case records in compliance with company and regulatory standards.

Monitor client progress through regular follow-ups and make necessary adjustments to care plans.

Collaborate with internal teams to improve service delivery and client outcomes.

Qualifications:

Bachelor's degree in Social Work, Psychology, Human Services, or a related field.years of experience in case management, social services, or a related field.

Excellent communication, problem-solving, and crisis intervention skills.

Familiarity with community resources and case management software.

Ability to work independently and within a team to support client needs.
