Schedule: Flexible; based on academic program and rotation requirements
Third Street Family Health Services, a Federally Qualified Health Center (FQHC), is accepting applications for clinical placement from students enrolled in medical and allied health programs, including LPN, RN, BSN, NP, PA, and MD/DO tracks. This placement offers hands‑on experience in a high‑impact, community‑based primary care setting with exposure to additional integrated service lines. Students will work under licensed clinical preceptors and gain valuable experience serving diverse, underserved populations.
Key Responsibilities:- Conduct patient interviews, vitals, and health screenings under supervision.
- Assist with patient care including preventive, acute, and chronic condition management.
- Observe and support medication management and treatment planning.
- Collaborate with interdisciplinary teams including behavioral health, dental, and case management staff.
- Document patient encounters in the Electronic Health Record (EHR) in accordance with clinical standards.
- Participate in quality improvement and care coordination initiatives as appropriate.
- Clinical supervision by licensed providers (MD, DO, NP, or PA).
- Experience in a collaborative, integrated care setting.
- Exposure to diverse patient populations and a range of physical health conditions.
- Opportunities for mentorship and professional development.
- Performance evaluations and letters of recommendation upon request.

Qualifications:
- Currently enrolled in an accredited medical or allied health program (LPN, RN, NP, PA, MD/DO).
- Seeking clinical rotation, externship, or internship for academic credit or licensure.
- Third Street must have an affiliate agreement on file with the school.
- Strong interpersonal skills and commitment to community health and health equity.
- Able to maintain professionalism and confidentiality in a clinical environment.
- BLS/CPR certification (as required by program).
- Completes credentialing requirements within first 90‑days of employment.
