Behavioral Health Services Case Manager; Part-Time

The Behavioral Health Services (BHS) Case Manager supports employees receiving behavioral health services through the DOE Hanford Site Occupational Medical Services program. This part‑time role focuses on care coordination, community resource referrals, patient education, and continuity of care for individuals navigating behavioral health concerns, return‑to‑work needs, leave‑related transitions, and Employee Assistance Program (EAP) services.

Working closely with behavioral health clinicians, medical case managers, Occupational Health staff, and community providers, the BHS Case Manager helps employees understand next steps, connect with appropriate resources, and successfully navigate services both within the clinic and in the community. The position also supports behavioral health education, outreach, records management, and departmental quality assurance activities.

• Identify behavioral health, psychosocial, socioeconomic, and other barriers that may affect an employee's ability to access care or successfully navigate workplace‑related health needs.
• Develop and maintain individualized case management plans, including goals, action steps, referrals, and follow‑up.
• Coordinate services among Occupational Health, behavioral health providers, community providers, and other appropriate resources.
• Support employees navigating return‑to‑work, leave‑related, and fitness‑for‑duty processes in coordination with the appropriate clinical and Occupational Health professionals.
• Assist EAP clients who need ongoing care with identifying and connecting to long‑term community‑based behavioral health services.
• Provide referrals and transition‑of‑care support for counseling, behavioral health, social services, and other community programs.
• Follow up with clients to monitor progress, address barriers, and help maintain continuity of care.
• Advocate for clients and help them understand treatment recommendations, available resources, and next steps.
• Build and maintain relationships with community providers and referral partners.

• Develop and maintain behavioral health education and community resource materials for employees, staff, and providers.
• Maintain an up‑to‑date network of behavioral health and community referral resources.
• Provide practical education and guidance related to care plans, available services, workplace wellbeing, and return‑to‑work needs.
• Assist with presentations and outreach activities for contractors and work groups on mental health, emotional wellbeing, and workplace wellness.
• Promote awareness of available behavioral health resources and encourage early connection to appropriate services.

• Support behavioral health records management, including release‑of‑information processing, document scanning, records organization, and related administrative functions.
• Maintain accurate, timely, and confidential documentation in accordance with clinic policies and applicable requirements.
• Assist with quality assurance activities related to behavioral health documentation, records, and departmental processes.
• Participate in departmental meetings, case reviews, and care coordination activities as appropriate.

Requirements

Required Qualifications

• Bachelor's degree in Social Work, Psychology, Human Services, or a related field

• At least two (2) years of relevant professional experience in case management, healthcare, behavioral health, community resource coordination, social services, or a related setting.
• Experience working with individuals navigating behavioral health, medical, psychosocial, or workplace‑related needs.
