Description
Under the general direction of the Supervisor/Manager of Clinic Operations with clinical oversight from the Director of Clinical Operations, the RN:
The Team Based Care (TBC) Care Manager RN independently uses the nursing process to care for a group of patients safely, therapeutically, and efficiently. This role conducts shared patient visits with providers as well as independent RN visits using standardized procedures/protocols and evidence‑based workflows. The TBC RN collaborates with other team members to coordinate and execute the plan of care and demonstrates initiative in the delivery of patient care in a Team Based Care Model.

Required Qualifications- Coursework/Training:
Graduate from an accredited school of Nursing. - Upon hire:
Washington Registered Nurse License. - 2 years Professional nursing experience.
- Bachelor’s Degree Nursing from an accredited educational institution.
- 1 year experience in ambulatory care nursing.

CompensationPosted are the minimum and the maximum wage rates on the wage range for this position. The successful candidate’s placement on the wage range will be determined based upon relevant job experience and other applicable factors. These amounts are the base pay range; additional compensation may be available for this role, such as shift differentials, standby/on‑call, overtime, premiums, extra shift incentives, or bonus opportunities.
