Responsibilities
- Assess, diagnose, and treat residents in the long-term care facility setting
- Develop personalized treatment plans for residents with mood disorders, thought disorders, and substance use disorders
- Serve as a leader in a multidisciplinary team to enhance residents’ overall well-being
- Conduct thorough behavioral health assessments and evaluations
- Develop and oversee individualized treatment plans with residents, families, and interdisciplinary teams
- Prescribe and manage appropriate medications and therapies tailored to the long-term care setting
- Provide counseling and support to residents and their families
- Monitor resident progress and adjust treatment plans as necessary
- Educate residents and staff about behavioral health conditions and available resources
- Master’s or Doctorate Degree in Nursing with a specialization in Psychiatric nursing
- Graduation from a state-certified advance practice program specializing in psychiatry/mental health
- At least 1-2 years of direct clinic care experience in behavioral health
- Proficiency in working with both adult and geriatric populations
- Specialty NP experience in Behavioral Health
- Strong interpersonal and communication skills
- Previous experience in psychiatric nursing before transitioning to NP
- Dedication to evidence-based practice and continuous professional development
- Must have a certification by the American Nurses Credentialing Center (ANCC) in Psychiatric Mental Health (PMH) as a Psychiatric Mental Health Nurse Practitioner (PMHNP)
- Must have an unrestricted license to practice as an advanced practice registered nurse (APRN) in the state of Ohio
- Must have a valid DEA number and unrestricted DEA license in Ohio
- Must have a state-issued Controlled Substance Registration (CSR) in Ohio
- Must be registered with the state Prescription Monitoring Program (PMP) in Ohio where direct services will be provided
- No suspension/exclusion/debarment from participation in federal health care programs (e.g. Medicare, Medicaid, SCHIP)
- Maintain credentials and licensure with all major insurance companies and licensing boards with the assistance of Provident credentialing staff
- Experience in delivering high-quality, patient-centered care
- Proficiency in documenting and navigating electronic health records (EHR) independently
- Ability to maintain strict client confidentiality
- Proficient computer skills, including working knowledge of Microsoft Office Suite
- Ability to work effectively on multidisciplinary team
- Ability to read, analyze and interpret general and professional documents
- Ability to write routine reports and correspondence
- Ability to effectively present information in one on one and small group situations
- Must possess the ability to make independent decisions when circumstances warrant such action
- Ability to foster teamwork; train and direct the work of team members and colleagues
- Excellent organizational skills, including the ability to work productively and make independent decisions
