Part time Registered Nurse Case Manager

Anticipated Territory:
Richmond Area

Schedule: Must be available for full days every Saturday and Sunday (4 SOCs/day). Also willing to consider full-time:
Saturday and Sunday plus 3 weekdays.

Utilize your clinical skills to complete the initial assessment of patients and determine home care needs, including:

• Conduct Start of Care (SOC) Admissions - OASIS
• Conduct follow-ups and evaluation visits as needed in designated territory
• Make home visits to patients in the designated territory
• Perform assigned duties, including reconciliation of medications, wound care, treatments, and procedures
• Monitor clients’ conditions, reporting changes to the clinical team
• Collaborate with physicians and agency staff to deliver high-quality care
• Accurately document observations, interventions, and evaluations related to patient care management and services provided
• Initiate and administer IV medications (antibiotics and diuretics) infusions safely and effectively in accordance with established protocols

  Dedicated resources to assist with administrative tasks

  Typically 5-15 mile radius for patient visits

• Current and unrestricted CA RN license
• Current BLS/CPR Certification
• Valid CA Driver's License
• Experience conducting assessments and SOC - OASIS is highly desired
• High level of clinical knowledge in home health care is preferred
• Experience with high-acuity patients is a plus
• Experience working with an interdisciplinary team is desirable
• Case Management fundamentals are desirable
• Previous experience with IV medication infusions is highly preferred
• Proficiency initiating IV therapy is a plus
• Self-directed, empathetic, critical thinker with outstanding interpersonal skills
• Leadership, organizational skills
