Part time Registered Nurse Case Manager
Anticipated Territory:
Richmond Area
Schedule: Must be available for full days every Saturday and Sunday (4 SOCs/day). Also willing to consider full-time:
Saturday and Sunday plus 3 weekdays.
Utilize your clinical skills to complete the initial assessment of patients and determine home care needs, including:
- Conduct Start of Care (SOC) Admissions - OASIS
- Conduct follow-ups and evaluation visits as needed in designated territory
- Make home visits to patients in the designated territory
- Perform assigned duties, including reconciliation of medications, wound care, treatments, and procedures
- Monitor clients’ conditions, reporting changes to the clinical team
- Collaborate with physicians and agency staff to deliver high-quality care
- Accurately document observations, interventions, and evaluations related to patient care management and services provided
- Initiate and administer IV medications (antibiotics and diuretics) infusions safely and effectively in accordance with established protocols

- Current and unrestricted CA RN license
- Current BLS/CPR Certification
- Valid CA Driver's License
- Experience conducting assessments and SOC - OASIS is highly desired
- High level of clinical knowledge in home health care is preferred
- Experience with high-acuity patients is a plus
- Experience working with an interdisciplinary team is desirable
- Case Management fundamentals are desirable
- Previous experience with IV medication infusions is highly preferred
- Proficiency initiating IV therapy is a plus
- Self-directed, empathetic, critical thinker with outstanding interpersonal skills
- Leadership, organizational skills
