Social Worker Supervisor
The Center for Elders’ Independence is a PACE (Program of All-Inclusive Care for the Elderly) organization that uses an interdisciplinary team approach to care planning and implementation to provide high‑quality, affordable integrated health care services to the elderly, including an Adult Day Health Center, and promote autonomy, quality of life, and the ability of individuals to live in their communities.
Position OverviewUnder the supervision of the Center Director, the Social Worker Supervisor is a mental health professional who helps frail and elderly PACE participants find solutions to a wide range of social problems. The role serves as an integral member of an Interdisciplinary Team (IDT), performs psychosocial assessments, develops and implements plans of care, and facilitates communication between participants, family, and CEI staff.
The supervisor is the location contact for the discipline and oversees the location’s Social Workers and Social Work Assistants.
Salary range: $92,150 to $138,225 per year, hourly rate commensurate with market, experience, skills, abilities and work history.
Duties and Responsibilities- Serve as the point person for the social work team at the assigned center.
- Participate in interview and hiring process with other leaders as directed.
- In conjunction with the Social Work Functional Manager, provide orientation and training to onboard incoming social workers and social work assistants at the assigned center.
- Collaborate with the Social Work Functional Manager across centers to ensure standardization of best practices or protocols in the department.
- Represent the Social Work department in quality and process‑improvement initiatives and meetings.
- Review and approve direct‑report time cards for accuracy and completeness.
- Ensure adequate coverage in the social‑work team during vacations and time‑off requests and approve or deny requests based on business needs.
- Ensure the social‑work team remains current with required assessments.
- Complete performance evaluations and participate in employee development and performance coaching.
- Organize psychosocial rounds in coordination with the Center team, psychiatrists, and/or CEI LCSW team.
- Support the standardization of best practices, supervision, and implementation of behavioral‑health training models and internal participant referrals.
- In conjunction with the Social Work Functional Manager, coordinate, manage, and evaluate student/intern placement with corresponding schools or universities as needed.
- Refer participants with severe mental challenges to behavioral‑health resources.
- Serve as an interdisciplinary team (IDT) member responsible for developing and implementing a comprehensive, integrated plan of care, monitoring progress toward objectives, and communicating participant changes to team members.
- Complete initial assessments, re‑assessments, and care‑plan updates for participants while continually evaluating their social‑service needs and those of the families.
- Make referrals to community resources, including those for financial management, benefits eligibility, and other related participant needs.
- Manage assigned caseload; prepare narrative and other reports, documents, and correspondence regarding participant status; document case files.
- Respond to, investigate, and report complaints of elder abuse.
- Conduct as‑needed family and/or caregiver conferences.
- Develop and maintain working relationships with other agencies such as the Department of Social Services, psychiatric facilities, hospitals, social agencies, and participate in hospital and/or SNF discharge planning.
- Maintain confidentiality of all company procedures, results, and information about participants, clients, or families in conformance with HIPAA principles.
- Make home visits, hospital visits, and travel to other places to meet with participants and service providers.
- Perform related duties as assigned.
- Master’s degree in Social Work from an accredited school of social work.
- 1–2 years of supervisory experience required; other related experience desirable.
