Outpatient Neuro Physical Therapist

Schedule: Day Shift, 40 hours per week, Monday–Friday, 8–10 hour shifts
Position OverviewThe Physical Therapist evaluates each patient and provides medically prescribed treatments and education to restore, improve, maintain optimal functioning, independence, and quality of life. The Physical Therapist also supervises the therapy services provided by Physical Therapist Assistants within the department.
Responsibilities- Evaluate each patient utilizing applicable diagnostic, prognostic, and functional ability assessments.
- Re‑evaluate patient status as appropriate for plan of care or change of condition.
- Interpret evaluation data, history, and medical documentation to determine rehabilitation potential and benefit from intervention.
- Identify physical, social, and psychological needs of the patient, family, and significant others.
- Utilize patient/family/caregiver input in setting functional rehabilitation goals.
- Document goals in functional, measurable terms with stated time frames.
- Incorporate information from evaluation and assessment to determine an appropriate, specific treatment plan.
- Involve patient/family/caregiver in treatment planning.
- Demonstrate competence in performing all procedures and treatments required in area of practice.
- Demonstrate awareness of continuity of patient care by following established plans.
- Modify patient care based on continual assessment and effect of intervention.
- Demonstrate skills in identifying, interpreting, and incorporating patient’s psychological, physical, and social responses to intervention.
- Provide and recommend equipment or supplies for home use.
- Provide for patient’s personal care and environment throughout treatment.
- Provide education appropriate to patient’s level of understanding regarding evaluation, treatment, and self‑responsibility.
- Instruct in home treatment program to facilitate rehabilitation and promote independence.
- Collaborate with patient, caregivers, and other health care professionals in planning discharge from rehabilitation services.
- Initiate discharge planning at time of initial evaluation.
- Confer and communicate with referring physicians and other appropriate persons through progress notes, letters, evaluations, team meetings, and direct contact concerning each patient’s proposed course, progress, and discharge plan.
- Complete clear, concise, and clinically useful documentation in the medical record in a timely manner according to policy.
- Provide clinical supervision to PTAs in the implementation of treatment plan.
- Counter‑sign PTA documentation in a timely manner in accordance with policy.
- Provide supervision at all times by direct contact (in person), Voalte, or phone during all hours of service provision.
- Provide direct supervision of holders of an IN temporary permit as defined by code.
- Provide clinical supervision to Physical Therapy and Physical Therapist Assistant students on site.
- Participate in QA&I activities.
- Ensure patient and departmental safety through work requests, safety procedures, and other appropriate measures.
- Supervise and direct all PTAs and rehab techs in provision of care, including progression of program to optimize outcomes.
- Assume responsibility to see that new PT orders are promptly scheduled and notify supervisor when needed.
- Conduct, participate in, and supervise training and observation of medical and nursing personnel, students, and other persons in PT.
- Market and promote PT services as appropriate.
- Assume responsibility for direction of PT services as assigned by manager or director.
- Submit capital equipment recommendations as appropriate.
- Inform manager and director of department needs.
- Understand and participate in appropriate volunteer opportunities within the community.
- BS, MS, or DPT degree or certification in Physical Therapy from an ACTA accredited program.
- Preference may be given to individuals with advanced clinical skilled experience or expertise.
- Must be fully licensed in Indiana to practice as a Physical Therapist. May work with a Temporary Permit under supervision of a PT per Indiana regulations.
- Additionally, if assigned to provide services in Ohio, the PT must hold a current Ohio license.
- Basic Life Support certification is required.
