About

The Role

As a Physical Therapist (PT) at Life Span Therapy, you will play a critical role in helping residents recover mobility, reduce pain, and improve their overall physical function following injury, surgery, or illness. You will assess residents' physical conditions, develop personalized treatment plans, and implement therapeutic interventions to promote healing and enhance quality of life. Your work will involve close collaboration with residents, families, and multidisciplinary healthcare teams to ensure comprehensive care and optimal outcomes.

You will also be responsible for monitoring resident progress, adjusting treatments as necessary, and educating residents on exercises and lifestyle modifications to support long-term health. Ultimately, your expertise will empower residents to regain independence and improve their physical well-being in a supportive and compassionate environment.

Job Responsibilities
  • Assures that Physical Therapy Services facilitate optimal resident outcomes and complies with the MDS procedures.
  • Assesses facility residents for PT needs at admission or when indicated to provide appropriate services.
  • Coordinates admission/discharge to Physical Therapy with the Interdisciplinary Team, Restorative Care Nurse and staff nurses.
  • Develop and implement individualized treatment plans based on resident needs and clinical best practices.
  • Performs therapy or assigns resident to a Physical Therapy Assistant (PTA) and communicates sufficient information about the treatment plan to nursing and other personnel to assure a team approach.
  • Monitor resident progress and document treatment outcomes accurately in compliance with healthcare regulations.
  • Educate residents and their families on injury prevention, health maintenance, and therapy techniques.
  • Utilize basic computer skills to manage resident records, schedule appointments, and communicate with the care team.
  • If needed, occasionally travels to other locations to provide physical therapy coverage during changes in demand or service delivery.
Minimum Qualifications
  • Must be a licensed Physical Therapist (PT) who is a graduate of an ACOTE accredited school.
  • Current state licensure to practice physical therapy.
  • Must have a valid Indiana Operator's license.
  • Demonstrated experience in resident care, including assessment and treatment planning.
  • Basic computer literacy for documentation and communication purposes.
Preferred Qualifications
  • Experience working in outpatient or rehabilitation settings.
  • Strong interpersonal and communication skills to effectively engage with diverse patient populations.
  • Continuing education or advanced training in infection control practices.
