Reid Health is seeking a Registered Nurse Care Coordinator to manage patient transitions and coordinate care across acute, post-acute and outpatient settings. You will educate patients, assess needs and collaborate with the care team and community resources to improve outcomes.
Ideal candidates have at least 3 years of acute care experience, Indiana RN licensure, and a strong focus on safety, communication and problem solving. This is a full-time, on-site role with regular weekday hours.
