• Active Pharmacist license in good standing in the state
• Minimum of 1 year of inpatient hospital pharmacy experience
• Experience with pharmacokinetic dosing (specifically vancomycin) and warfarin management
• Experience with Epic Willow Inpatient is preferred but not required

• Dispensing medications and consulting on drug therapy
• Monitoring patient responses to therapy and adjusting medications as necessary
• Providing pharmacotherapy consultation to healthcare professionals
• Ensuring compliance with regulations and best practices in medication management
