Position Overview
The Registered Nurse (RN) provides skilled home health care to patients across the Richmond, KY service area. This role is centered on delivering high quality, patient-focused care through comprehensive assessments, accurate OASIS documentation, and care plan management. The RN will collaborate closely with physicians and interdisciplinary teams to ensure optimal patient outcomes while maintaining compliance with all regulatory standards.
Schedule & Work Details- Weekly Guarantee: 32 hours per week (25 productivity points)
- Schedule: Day shift
- On Call Requirement: Minimum of 1 weekday per week and 1 weekend per month
- Pay Rate: $51 to $53 per hour
- Mileage Reimbursement: Yes, from first patient to last patient visit
- Dress Code: Royal Blue or Black scrubs
- Perform comprehensive nursing assessments including Start of Care, recertification, and discharge visits
- Develop, implement, and update individualized care plans in collaboration with physicians and care teams
- Provide skilled nursing services in accordance with physician orders and patient needs
- Complete OASIS documentation accurately, thoroughly, and within required timelines
- Ensure all documentation meets agency, state, and payer requirements
- Maintain compliance with home health regulations and clinical standards
- Collaborate with interdisciplinary teams to coordinate patient care and services
- Communicate effectively with patients, families, physicians, and team members
- Monitor patient progress and adjust care plans as needed to improve outcomes
- Maintain a high level of professionalism and accountability in all patient and team interactions
- Participate in team meetings, case conferences, and performance improvement initiatives
- Demonstrate flexibility to meet patient care demands, including on call rotations
- Active Registered Nurse (RN) license
- Minimum 3 years of home health experience
