Job Overview
This role functions as the liaison and communicator with the patient, caregivers, healthcare providers, and multidisciplinary team members, including post‑acute care and third‑party payers. The candidate discusses alternative care options with patients, caregivers and the team, assists with discharge planning, and facilitates movement along the healthcare continuum to ensure quality, cost‑effective outcomes.
Key Responsibilities- Provides continuity of care by ensuring smooth transitions between care settings.
- Develops relationships with patients and the multidisciplinary team to navigate subsequent treatment and follow‑up, reflecting continuity of care.
- Manages high‑risk, complex patient care to minimize readmission.
- Works closely with physicians to coordinate patient care plans and maintain up‑to‑date, patient‑centered care plans across the team.
- Communicates with all team members as a patient advocate.
- Prepares, executes, and reinforces post‑discharge care plans.
- Identifies barriers to care to elicit process changes for patients navigating the continuum of care.
- Collaborates with leadership to review processes and improve the clinical experience for referred patients and the referring physician.
- Adapts behavior to specific patient populations, including privacy respect and communication style.
- Performs other related duties as required.
- Graduate of an accredited school of nursing.
- Preferred:
Associate’s or Bachelor’s degree in nursing. - At least 2 years of nursing experience.
- Current registered nurse (RN) license in the state of practice.
- Basic Life Support (BLS) certification from the American Heart Association.
- Certification in a clinical specialty area.
- Proficiency in using computers, software, and web‑based applications.
- Effective verbal and written communication skills with the ability to present information clearly and professionally to varying levels of individuals across the patient care process.
- Excellent conflict resolution skills.
- Ability to work a flexible schedule, including 24/7, weekend, holiday, and on‑call availability, and to travel between facilities.
The role requires occasional lifting of 20–50 pounds and frequent lifting of 10–25 pounds, and may involve exposure to blood, bodily fluids, tissue, hazardous medications, and waste. The incumbent must perform tasks in a patient care area and may need to work a moderate physical workload, including standing, walking, and using arm or leg controls for brief periods.
