Position: Licensed Practical Charge Nurse-Full Time/Night Shift

Purpose

Under the general supervision of the Director of Nursing, the LPN Charge Nurse supervises, provides, and directs nursing care in compliance with company and facility policies and procedures, complying with all applicable federal, state, and local regulations. The role involves advanced level observations, reporting, recording, forming nursing judgments, and carrying out nursing actions based on scientific principles.

Essential Functions
  • Supervise, provide and direct nursing care in compliance with facility policies and procedures.
    • Make advanced level observations; report and record observations appropriately; form nursing judgments and perform nursing actions.
    • Follow established standards of nursing practices.
    • Assume responsibility for compliance with federal, state and local regulations.
    • Interpret existing policies for staff, families and physicians.
    • Perform daily rounds and develop/implement accurate comprehensive care plans based on each resident’s needs.
    • Follow policies and procedures for passing medications and treatments; order and credit medications as needed; keep accurate accounting of all medications (stock medications, unit dose, and narcotics).
    • Follow staffing guidelines and make nursing assignments.
    • Receive and give accurate reports on resident condition to ensure continuity of care.
    • Monitor diets and supervise meal trays and snacks; assist in passing meal trays, feeding, and passing snacks.
    • Follow facility admissions procedures and communicate admissions to other departments, staff, physicians and service providers.
  • Supervision
    • Ensure employees follow the chain of command in a manner consistent with personnel policies.
    • Assign nursing assistants and ensure efficient completion of their assignments.
    • Supervise, document, and correct poor performance of nursing assistants.
    • Keep Unit Managers/Director of Nursing informed of poor performance problems on a timely basis.
    • Participate in performance evaluation of nursing assistants and provide counseling, coaching and training.
    • Present performance evaluations to staff.
  • Communicate professionally and effectively
    • Follow written and verbal directions.
    • Provide exemplary customer service to residents, peers, families, managers, and visitors.
    • Report any changes in physical or mental conditions on a timely basis.
    • Follow all company, facility and state policies and procedures.
    • Attend staff meetings, in‑services, inter‑disciplinary team and committee meetings as assigned.
    • Exercise keen observations of noise, odors, cleanliness, etc.
    • Cooperate with other staff members in all departments to ensure quality environmental conditions for the residents and staff.
  • Document resident information timely, accurately and confidentially
    • Document changes and notifications to POA and doctor; perform comprehensive assessments as assigned.
    • Verify change‑over MARS/TARS are correctly updated and new MAR/TAR changes are made by the 1st of each month.
  • Adhere to Resident Rights documentation
    • Know resident rights and help residents exercise/protect their rights.
    • Report resident complaints to supervisor in charge, maintaining confidentiality.
  • Adhere to Safety & Compliance policies and procedures
    • Ensure residents are safe (e.g., Wander guard tabs securely attached, bracelets working properly).
    • Demonstrate proper use of equipment; report equipment needs or repairs.
    • Use protective equipment and follow infection control protocol standards.
    • Practice universal precautions and life safety measures.
  • Expectations
