Purpose:

** The purpose of this position is to supervise, provide and coordinate nursing care in compliance with community policies and procedures and to assess residents' care needs and take appropriate action.
** Primary Duties (Essential Functions) include but are not limited to:
*** Follow established standards of nursing practices and implement community protocols and procedures.
* Interpret existing policies and procedures to nursing assistants, restorative nursing assistants, residents, families and physicians.
* Supervise and evaluate direct resident care and initiate corrective action as necessary.
* Obtain report from nurse being relieved and follow-up as necessary. Provide report to nurse coming on duty, including sufficient information for follow-up action as necessary.
* Report staffing needs to supervisor.
* Conduct resident rounds daily, reporting problems to nursing supervisor and initiating corrective action.
* Identify safety hazards and initiate corrective action.
* Identify needs of resident and initiate care needs as indicated.
* Follow, supervise and evaluate implementation of the Residents' Rights.
* Participate in community education programs.
* Provide supervision to Nursing Assistant providing direct resident care.
* Attend staff meetings, interdisciplinary team meetings, rehabilitation meetings, in- service classes and committee meetings as assigned.
* Follow daily staffing assignment sheets and notify supervisor if staffing needs to be adjusted.
* Assume responsibility for compliance with state and local regulations within the assigned area of responsibility.
* Counsel Nursing Assistants and communicate issues to supervisor or other leadership as necessary.
* Administer and document as indicated, direct resident care, medications and treatments per physicians' orders and record care provided as indicated.
* Perform assessment of residents as assigned and record as indicated.
* Develop and implement a care plan based on each resident's needs and his/her assessment as indicated.
* Promptly report changes in residents' conditions to the physician, director of nursing and responsible party, and take follow-up action as necessary.
* Follow community admission procedures and communicate resident admission to other departments, physicians and ancillary service providers.
* Competently perform basic nursing skills.
* Follow community discharge /transfer procedure and coordinate discharge/transfer with other departments, physicians and ancillary service providers.
* Complete documentation related to the resident's discharge/transfer.
* Supervise response to resident's call for assistance.
* Listen to resident, family, or physician concerns and initiate action and/or report concerns to DON.
* Count narcotics, check emergency kit and exchange keys at the beginning and end of each shift.
* Check meal trays and nourishment to ensure proper diet is served and residents are assisted as necessary.
* Supervise the application and removal of restraints and devices used to position and enable residents to function as independently as possible.
* Receive physician orders, receive and store medications appropriately.
* Notify physician, when necessary, of changes in residents' conditions, weight, medication errors, medication not available, abnormal laboratory/X-Ray results and any other unusual observations.
* Document resident progress notes as indicated.
* Work cooperatively with residents, nursing staff, physicians, families, consultant personnel, community agencies and ancillary service providers.
* Obtain continuing education required to maintain licensure. Follow written and oral directions.
