Position: Hospice After-Hours Registered Nurse (RN)

Part-Time Opportunity (20 hours per week)

Schedule:

Varied shifts during department operational hours which include 4p-8a Monday - Friday and 24 hours on weekends and observed holidays. Required every other weekend and two observed holidays per year. Compensation: $37.50/hour

HOW YOU'LL MAKE A DIFFERENCE

As a Part-Time Triage Registered Nurse (RN) - Hospice the work you do every day makes a difference in the lives of our patients. Our nurses give our patients the greatest gift – the ability to spend enhanced quality time with their loved ones in their preferred environment.

HOW YOU'LL WORK

You’ll work a set schedule on a 2-week rotation this will include after hours, weekends and holidays to triage calls, provide symptom management advice and patient-care intervention, identify resources, consult and refer, and assign nurse follow-up activities as needed.

MAJOR AREAS OF RESPONSIBILITY
  • Patient Care:
    Provide triage services to patients and follow-up calls to assess efficacy of recommended treatments. Assist field staff with clinical issues during scheduled work time.
  • Communication:
    Communicate significant findings and collaboration of problems, patient’s needs, reports changes in condition or environment to the physician, clinical manager and members of the hospice care team and obtains physician orders as required other to ensure continuity and quality of care.
  • Teamwork:
    Serve as a clinical expert and management resource to the weekend/evening clinical teams. With Clinical Supervisor assistance, reassign patient visits in the event of field staff call-in during off hours and holidays as needed.
  • Family Educator/Advocate:
    Provide an exceptional patient experience through clinical, educational and emotional support to patients/families.
  • Policies:
    Follow current hospice nursing professional standards of practice and agency approved protocols. Provide accurate timely documentation of patient services, such as updates the Plan of Care, physician orders, MAR/Pharmacy profile, within HCHB to reflect the individualized plan of care and outcomes of interventions in accordance with company standards and federal, state, and local standards, guidelines, and regulations.

  • Compassionate communicator with a positive attitude.
  • Patience is a virtue when working with patients, families, physicians, and coworkers.
  • Attention to detail is critical, as is being observant and following directions.
REQUIREMENTS
  • Multi-state RN Licensure (currently OH and MI) with current licensure to practice professional nursing.
  • May require additional licensure as company grows.
  • Three years of experience within the last five (5) years in a hospice or related health care organization.
  • Valid driver’s license and auto insurance in your name as a driver.
  • Capable of all physical demands.
HEALTH QUALIFICATIONS
  • Health Requirements:
    Must be able to meet all physical position requirements. Evidence required of a current negative tuberculin skin test or x-ray or receives TB testing at the date of orientation. Thereafter an annual tuberculin skin test is required, unless the employee has documentation of a previous TB test. Employee must be free of physical/medical conditions, which would limit or restrict their ability to perform the job functions listed below.

    Bloodborne exposure risk is all.
