The RN implements professional nursing practice centered on the patient as a whole person. The RN creates a caring environment focused on healing and wellness. Nursing practices are based on evidence and current research along with hospital policies and procedures. Clinical decision-making incorporates all phases of the nursing process, critical thinking, and creativity. The RN promotes nursing excellence through helping relationships among staff and across other disciplines.

The RN coordinates, ensures, and provides direct patient care activities on the unit. Responsibilities include provision of nursing rounds, assessments, and treatments. The RN facilitates admission, transfer, and discharge activities. The RN provides patient and family-centered education, care coordination, and individualized treatment planning as applicable. The RN performs and documents a comprehensive assessment in accordance with the RN's scope of practice.

The RN considers the patient's culture, age, and developmental level. Identifying patient risk factors and presenting complaints is essential. The RN collects objective and subjective data. The RN verifies initial admission assessments, if obtained by other healthcare team members. A problem list is developed, prioritizing patient needs based on input from the inter-professional team. Actions and interventions are designed relevant to the patient's culture and age.

The RN determines desired outcomes in coordination with the patient and family. Patient care therapies and procedures are fully implemented with empowerment within the scope of practice. The RN explains procedures, treatments, and processes to the patient and family. The RN initiates action to alleviate patient stress and discomfort. Recognizing and responding to changes in patient condition appropriately is critical.

The RN conducts and documents ongoing assessments and responses to procedures and therapies. Tasks are delegated appropriately and team members are overseen in patient care activities. The RN monitors the accuracy and completeness of delegated care. Educational needs of patients related to prioritized problems are identified and addressed. The RN facilitates the patient discharge process by executing discharge orders in a timely manner.

Input into patient/nurse assignments is provided in consideration of available staffing and patient acuity. Active participation in unit-based activities in support of organizational values and priorities is demonstrated. The RN promotes a positive group dynamic and healing environment through words and actions. Nursing excellence is assured in accordance with accepted standards, evidence-based practice, and a caring philosophy. Participation in department and/or unit quality improvement activities and core measures is encouraged.
