The RN implements professional nursing practice centered on the patient as a whole person. The focus is on the creation of a caring environment centered on healing and wellness. Nursing practices are based on evidence and current research, as well as hospital and department policies and procedures, regulatory standards, and professional guidelines. Clinical decision-making incorporates all four phases of the nursing process, critical thinking, and creativity.
Nursing excellence is promoted through helping relationships among nursing staff and across other disciplines. The RN coordinates, ensures, and provides direct patient care activities on the unit. Responsibilities include provision of nursing rounds, assessments, and treatments. Facilitating admission, transfer, and discharge activities is essential. Provision of patient and family-centered education is a key component of care. Care coordination and individualized treatment planning are performed as applicable.
The RN conducts a comprehensive assessment in accordance with scope of practice. Consideration of the patient's culture, age, and developmental level is critical. Identifying patient risk factors and presenting complaints is essential. The RN collects both objective and subjective data for assessment. Verification of initial admission assessment is performed as necessary. A problem list is developed, prioritizing patient needs based on input from the inter-professional team.
Actions and interventions are designed relevant to the patient's culture and age. Desired outcomes are determined in coordination with the patient and family. Implementation of patient care therapies and procedures is fully executed within scope of practice. The RN explains procedures, treatments, and processes to the patient and family. Initiating action to alleviate patient stress and discomfort is a priority.
Recognizing, responding to, and communicating changes in patient condition appropriately is crucial. Ongoing assessment and response to procedures and therapies are documented. Delegation of tasks is done appropriately, overseeing team members in patient care activities. The RN monitors the accuracy and completeness of delegated patient care. Identifying and addressing educational needs of patients is important. The RN facilitates the patient discharge process by executing discharge orders timely.
Patient and nurse assignments are made considering available staffing and patient acuity. Participation in unit-based activities in support of organizational values and priorities is encouraged. The RN promotes a positive group dynamic and healing environment. Assuring nursing excellence is in accordance with accepted standards and evidence-based practices. Active participation in quality improvement activities is expected. A holistic perspective regarding patient care is utilized.
