The RN implements professional nursing practice centered on the patient as a whole person. The focus is on creating a caring environment focused on healing and wellness. Nursing practices are based on evidence and current research as well as policies and procedures, regulatory standards, and professional guidelines. Clinical decision-making incorporates all four phases of the nursing process, critical thinking, and creativity.
Nursing excellence is promoted through helping relationships among nursing staff and across other disciplines. The RN coordinates, ensures, and provides direct patient care activities on the unit. This includes the provision of nursing rounds, assessments, and treatments. Facilitating admission, transfer, and discharge activities is also part of the role. The RN provides patient and family-centered education, care coordination, and individualized treatment planning as applicable.
The RN performs and documents a comprehensive assessment in accordance with the RN's scope of practice and hospital policies. Consideration is given to the patient's culture, age, and developmental level. The RN identifies patient risk factors and presenting complaints. Patient health information is obtained from family or others as necessary. Objective and subjective data is collected for assessment. The RN develops a problem list, prioritizing patient needs based on input from the inter-professional team.
Actions and interventions relevant to the patient's culture and age are designed. The RN determines desired outcomes in coordination with the patient and family. Patient care therapies and procedures are fully implemented within the scope of practice. Procedures, treatments, and processes are explained to the patient and family. The RN initiates action to alleviate patient stress and discomfort. Changes in the patient's condition are recognized, responded to, and communicated appropriately.
Ongoing assessment and response to procedures and therapies are documented according to hospital policies. Tasks are delegated appropriately, and the RN oversees team members in the performance of activities regarding patient care. The accuracy and completeness of delegated patient care are monitored. The RN identifies and addresses educational needs of patients related to prioritized problems. Facilitating the discharge process includes executing discharge orders in a timely manner.
The RN provides input into patient/nurse assignments considering staffing and patient acuity. Active participation in unit-based activities supports organizational values and priorities. A positive group dynamic and healing environment are promoted through words and actions. Nursing excellence is assured in accordance with accepted standards and evidence-based practice. Participation in department and/or unit quality improvement activities is expected. A holistic perspective regarding patient care is utilized throughout the role.
