Intensive Case Manager for Adults - Carlisle, PA

• Travel throughout Cumberland and Perry Counties to meet clients in their homes and communities.
• Deliver Intensive Case Management (ICM) services to adults with serious mental health needs.
• Conduct assessments, develop service plans, and provide ongoing support through linking, monitoring, advocacy, and skill-building.
• Support clients in accessing psychiatric care, community resources, and social supports.
• Provide education and support to families and significant others.
• Maintain accurate and timely documentation in compliance with Merakey Behavioral Health, County, and State regulations.
• Participate in clinical supervision, staff meetings, and quality improvement initiatives.
• On-call for caseload.

• Bachelor's degree in sociology, social work, psychology, or a related field; OR high school diploma/GED with 12 college credit hours completed in relevant coursework, plus two years' experience in human services with one year in direct client contact; OR a registered nurse (RN) license.
• Valid driver's license and reliable transportation required.
• Experience working with adults in public or private human services is preferred.
