Our Physicians support our enrolled geriatric participants Monday through Friday to provide comprehensive medical care across a variety of settings, including our Day Center clinic, participants' homes, nursing homes, assisted living facilities, and personal care boarding homes. Most visits occur at the Innov Age Center clinic, though some physicians may specialize in nursing home care. Close coordination with hospital-based providers is essential.

• Provide medical care to participants, including evaluation, diagnosis, and management of medical conditions; take medical history, perform physical examinations, order and interpret diagnostic tests, and formulate assessment and treatment plans.
• Write prescriptions, follow up on treatments, and manage medical information from specialists and hospital teams.
• Document medical care in the participant’s medical record in compliance with Innov Age PACE documentation policies, and contribute to the development and enhancement of electronic medical records.
• Participate on a rotational basis in after‑hours on‑call coverage; triage is performed first by a registered nurse, with only a subset of calls (
