Position OverviewThe Emergency Department Clinical Social Worker 2 delivers discharge planning and social work services during normal business hours and the entire hospital after hours. This role requires interaction with a variety of patient populations including neonatal, pediatric, adolescent, adult, and geriatric. The position provides direct intervention with patients and families and, in collaboration with the health care team, offers assessment, problem solving, advocacy, community referrals, and crisis intervention services.
The candidate is responsible for the daily management of all complex discharge planning and social work functions.
- Licensed Clinical Social Worker in the State of Virginia preferred
- Licensure, Certification, or Registration Requirements for continued employment: N/A
- Minimum of one (1) year of clinical social work experience
- Clinical social work experience in an acute care setting (preferred)
- Related experience in acute discharge planning, home health, case management, long‑term care, or related community agency (preferred)
- Master's of Social Work Degree from a Council on Social Work Education accredited program (required)
- Education/Training Preferred: N/A
Primary responsibilities include proactive development, implementation, and ongoing revision of an interdisciplinary plan of care for patients who receive care in the Emergency Department. The Clinical Social Worker negotiates and collaborates with the interdisciplinary team, community resources, and payers to facilitate timely and cost‑effective discharge plans that enhance appropriate health and social services in the continuum of care.
