About the Role

We are seeking a dedicated PRN
Physical Therapist (PT) with an interest or expertise in neurological rehabilitation to join our clinical team. In this role, you will perform examinations, evaluate client mobility, and design evidence-based intervention strategies for individuals recovering from or managing neurological conditions, injuries, or developmental disabilities.

Working in a supportive, interdisciplinary environment alongside OTs, SLPs, case managers, and nursing, you will empower clients to restore functional movement, build independence, and integrate into community and vocational settings.

Key Responsibilities

Clinical Evaluation & Care Planning

  • Perform comprehensive PT examinations including patient history, systems review, and standardized tests and measures.
  • Diagnose movement dysfunctions and establish individualized plans of care focusing on transfer training, gait, bed mobility, and community mobility.
  • Re-evaluate patient progress regularly, modifying interventions and developing structured discharge plans.

Direct Treatment & Rehabilitation

  • Deliver therapeutic interventions to promote mobility, reduce pain, restore function, and prevent long-term disability.
  • Prescribe, fit, and train clients and families on adaptive equipment, compensatory strategies, and home modifications.
  • Provide practical training in cardiovascular conditioning, individual fitness programs, and vocational or avocational task adaptation.
  • Train caregivers, family members, employers, and care team members to support the client's treatment and discharge goals.

Interdisciplinary Collaboration & Compliance

  • Participate in interdisciplinary team meetings to coordinate holistic care and maintain alignment with client goals.
  • Complete clinical documentation (evaluations, session notes, monthly progress reports) in compliance with state regulations, DDD, HIPAA, and CARF standards.
  • Consult with external healthcare providers and physicians to coordinate comprehensive treatment strategies.
  • Maintain professional licensure and participate in continuing education.

Qualifications & Requirements

  • Education: Graduate of an accredited Physical Therapy program (DPT required for recent graduates; Master's or Bachelor's accepted for legacy licensees).
  • Licensure: Current, active state Physical Therapist (PT) License (or eligibility).
  • Experience: Clinical experience treating neurological conditions in a community-based or outpatient setting is strongly preferred
    . Experience working within CARF-accredited programs is a plus.
  • Operational Requirements: Valid driver’s license with a record of responsible driving and ability to lift up to 50 lbs.
