Are you an experienced Registered Nurse (RN) with a passion for providing exceptional care in a home health setting? Our fast-growing, dynamic home health agency is seeking an experienced RN Case Manager to join our team. This position offers a chance to be part of an exciting, expanding company with opportunities for advancement, including the possibility of a management position in the near future.

• Manage and coordinate care for patients in their homes, ensuring the delivery of high-quality, compassionate care.
• Develop, implement, and evaluate individualized care plans.
• Work closely with physicians, families, and the interdisciplinary team to ensure the best patient outcomes.
• Provide clinical assessments, monitoring, and education to patients and their families.
• Supervise home health aides and support staff as needed.
• Ensure compliance with all regulatory requirements and maintain accurate documentation.

• Active and unrestricted RN license.
• Minimum of 2 years’ experience in home health or case management.
• Strong clinical assessment, critical thinking, and communication skills.
• Ability to work independently, with excellent time management skills.
• BLS/CPR certification required (other certifications preferred).
• Management experience a plus, but not required.
