The MDS Coordinator

Will coordinate, record, update, and monitor all Minimum Data Set (MDS) documentation in accordance with current federal, state and local regulations. Coordinate the development of an individualized Plan of Care.

Major Accountabilities/Essential

Job Duties
  • Apply accurate MDS codes according to RAI guidance, federal and state regulations, the Center for Medicare and Medicaid Services regulations (CMS), and internal policies and procedures.
  • Coordinate the interdisciplinary completion of the MDS and attest to the accuracy of sections completed; transmit completed MDS to CMS timely.
  • Develop and coordinate individualized interdisciplinary plans of care.
  • Partner with all members of the interdisciplinary team to validate accuracy of documentation to support the Minimum Data Set (MDS) and Care Area Assessments (CAA’s).
  • Apply basic knowledge of ICD
    10-CM coding as required.
  • Use and adapt to new technologies, including tools, equipment, methods of application, computerized systems and processes, etc., to impact productivity of department and services; keep job-required technical skills up to date.
  • Maintain open communication with nursing staff to report any changes in status or condition of residents.
  • Analyze assessment data in order to capture the appropriate RUG category for proper reimbursement.
  • Additional duties as assigned.
Physical Requirements
  • Ability to lift 25 pounds without assistance, 26+ pounds with manual or mechanical assistance.
  • Ability to sit, stand, lift, bend, reach, push and pull to meet the needs of residents.
Requirements
  • Associate’s degree in nursing or equivalent combination of education and experience to fulfill duties and responsibilities of position.
  • Valid license as a Registered Nurse in the State of Virginia in good standing.
  • Five years clinical nursing experience; two plus years’ experience with PPS/CMS in a Medicare Nursing Home setting.
  • CPR, AED and First Aid Certification.
REQUIRED EDUCATION AND EXPERIENCE
  • Associate’s degree in nursing or equivalent combination of education and experience to fulfill duties and responsibilities of position.
  • Valid license as a Registered Nurse in the State of Virginia in good standing.
  • Five years clinical nursing experience; two plus years’ experience with PPS/CMS in a Medicare Nursing Home setting.
  • CPR, AED and First Aid Certification.
Desired Education And Experience
  • Bachelor’s degree in nursing.
  • Registered Nurse Assessment Coordinator certification.
