The Transplant Coordinator delivers and accurately documents direct, age‑specific care to a patient or group of patients based on medical orders and an integrated, multidisciplinary plan of care. This job safely administers prescribed medications, identifying incompatibilities and/or interactions. This job manifests a high level of expertise in the assessment, coordination and care of the complex patients, families, or communities to actual or potential health problems;
prevention of illness and injury; maintenance of wellness; and provision of comfort.
The Transplant Coordinator demonstrates strong clinical experiences in patient care. The position demonstrates a high level of expertise in the assessment, diagnosis, and treatment of the complex patients and their responses to actual or potential health problems. The position assists the health care team with the prevention of illness and injury, maintenance of wellness, and provision of comfort. The position also provides direction, education, and leadership.

Current RN licensure in Virginia or eligible.
Licensure, Certification, or Registration Requirements for continued employmentCurrent RN licensure in Virginia, AHA BLS HCP Certification or equivalent.
Experience Required- Minimum of three (3) years of nursing experience in transplant, critical care or medical‑surgical nursing
- Two (2) years of progressive leadership experience
- Demonstrated clinical expertise for population served with a minimum of one (1) year in specialty area
- Five (5) years of clinical experience in the transplant field Academic Healthcare experience
- Advanced Practice Nursing Role or Clinical Leadership experience
Baccalaureate Degree in Nursing from an accredited School of Nursing.
Education/Training PreferredMasters Degree in Nursing or Certification in Transplantation.
Independent Action(s) Required- Follows documented physician/licensed independent provider orders, within the boundaries of the Nurse Practice Act and established standards, policies and practice guidelines.
- Organizes and plans work with input from the patient/family with specific outcomes and demonstrates use of sound clinical judgment and resources.
- All day-to-day operations of the role are handled independently with minimal assistance.
- Advice and guidance are sought from the Manager or Director on an as-needed situational basis and when there is a major impact on activity outside the unit.
- Actions that are taken without prior approval include daily operational functions and leadership of staff.

- Emotional: fast pace environment, able to handle multiple priorities, frequent and intense customer interactions, able to adapt to frequent change
- Coordinator volume of approximately 300 patients after kidney or kidney/pancreas transplant
- Support in the transplant clinic Monday-Friday for patients transplanted and followed annually per UNOS regulations
- Coordination of outside lab draws (Lab Corp, Quest diagnostics, etc.)
- Patient education of medication use and dose changes
- Triage patient clinical calls
- Coordinate follow-up care with inpatient transplant team
- Monitor progress and ongoing treatment of post‑transplant recipients
