Nurse Practitioner – Sign on Bonus!

We’re looking for a Nurse Practitioner to join a respected care team in a supportive, patient-focused setting in Richmond, VA
. This full-time position is ideal for someone seeking stability, autonomy, and a balanced Monday–Friday schedule with no nights or weekends.

• Provide primary care services to seniors and patients with complex health needs, a mix of disease management, medication management, and chronic care.
• Conduct clinical assessments and develop personalized care plans in assisted living, independent living, and skilled nursing facilities.
• Collaborate with interdisciplinary teams to ensure seamless care transitions and improved patient outcomes while managing chronic conditions and acute visits with a focus on preventative health.

• $120K–$140K Base Salary
• $5K Sign On Bonus
• Quarterly performance bonus based on metrics (patient volumes, quality, documentation timeliness)
• Mileage Reimbursement for travel between facilities

Monday – Friday | 8:00 am - 5:00 pm
