Responsibilities- Conduct adult or geriatric assessments that include medical history, diagnosis and treatment, health education, physician referrals, case‑management referrals, follow‑up, and documentation according to Matrix guidelines and protocols.
- Administer and order point‑of‑care and lab‑based testing as needed.
- Collaborate with physicians, case managers, social workers, family members, key caregivers, and ancillary medical personnel as appropriate.
- Work with the Primary Care Physician (PCP) on patient education and follow‑up as necessary.
- Provide services at the member’s place of residence, which may be a home or Skilled Nursing Facility.
- Flexibility – Visits are scheduled based on your availability. Assessments occur Sunday‑Saturday from 8 am to 7 pm, allowing daytime, evening, and weekend work.
- Competitive Pay – Earn income based on the number of assessments completed. Those completing 20–100 visits per month can earn $1,800 to $11,500 per month, with per‑assessment rates of $90 to $115 depending on state.

- Education and Support – Comprehensive onboarding, preceptor assignment, 1:1 feedback, and free access to continuing education.
- Additional Licensure – Matrix supports obtaining additional state licensure and credentialing in neighboring states as business needs arise.
- Full‑time Opportunities – PRN providers can transition to full‑time roles supporting their home location, a state, or region and receive a full‑time sign‑on bonus.
- Master’s Degree or equivalent experience and completion of NP or PA licensure.
- Current NP or PA licensure in state of practice, including prescription authority or eligibility to obtain it.
- Board certified by the NCCPA, AANP, ANCC, or AACCN in Adult, Adult Gerontology Primary Care, Adult Gerontology Acute Care, Family, Emergency, or Acute Care specialty.
- Current BLS, ACLS, or CPR certification.
- One year experience as a Nurse Practitioner or Physician Assistant preferred; new graduates are encouraged to apply.
- Valid state driver’s license and proof of adequate automobile insurance coverage.
- Strong computer skills and familiarity with employee health/medical record software.
- Excellent verbal and written communication skills.
