OverviewVCU Health is seeking authentic, passionate and inspiring candidates for our virtually enabled Hospital at Home and Transitional Remote Patient Monitoring (tRPM) program. Responsibilities- Ensure quality successful enrollment of patients into the tRPM program.
- Manage a referral work queue.
- Conduct bedside education on the tRPM program and equipment with patients prior to hospital discharge.
- Enroll patients in the tRPM technology platform.
- Assist the healthcare team with prevention of illness and injury, maintenance of wellness, and provision of comfort.
- Provide direction, education, and begin to assume leadership responsibilities at the unit level.
- Deliver and accurately document direct, age‑specific care to a patient or group of patients based on medical orders and an integrated, multidisciplinary plan of care.
- Safely administer prescribed medications, identifying incompatibilities and/or interactions.
Transitional Remote Patient Monitoring is a 30‑day post‑discharge program that supports patients utilizing vital sign monitoring, nurse triage, provider telehealth visits, and nurse navigation.
Licensure, Certification, or Registration Requirements (Hire)Licensure, certification or registration varies depending on the incumbent's employment background and transitional assignment; LPN licensure in Virginia or eligible.
Licensure, Certification, or Registration Requirements (Continued Employment)Current LPN Licensure in Virginia, AHA BLS Certification or equivalent.
Experience- Required:
Varies based on transitional duty assignment. - Preferred:
Experience as an LPN in either an acute care setting or applicable area of practice.
- Required:
Graduate from a Board of Nursing approved School of Practical Nursing. - Preferred: N/A.
Follows documented physician/licensed independent provider orders, standards, and practice guidelines such as treatments and organizes work to carry out assignment.
AdditionalPosition Requirements
- Licensed Practical Nurses work all three shifts, weekends and holidays.
- Individual shift rotation and schedules will be established with your specific Nurse Manager/Practice Manager.
- Schedules, including requirements of shift rotations and hours of work may be adjusted as necessary to meet unit requirements and/or as needed.
Physical:
Lifting 50-100 lbs.;
Exposure to potentially hazardous and infectious substances.
Activities:
Prolonged standing, prolonged sitting, frequent bending, walking (distance), climbing (steps, ladder, other), reaching (overhead, extensive, repetitive).
Mental/Sensory:
Strong recall, reasoning, problem solving, hearing, speak clearly, write legibly, reading, logical thinking.
Emotional:
Steady pace, frequent and intense customer interactions, noisy environment, able to adapt to frequent change.
