Job Description

As a Licensed Practical Nurse (LPN) working with Intensa Care, you will be responsible for providing personalized, one-on-one care to individuals with developmental disabilities. You will ensure that each patient receives high-quality, person-centered care, and work closely with their family, and our team of healthcare professionals. Each shift, you will be supported by a Certified Nursing Assistant (CNA) or Direct Support Professional (DSP), who will be present in the home to assist you.

Key Responsibilities
:

  • Provide direct, 1:1 patient care for individuals with developmental disabilities (DD) under the supervision of a Registered Nurse (RN).
  • Administer medications, treatments, and prescribed procedures.
  • Monitor and assess patients' vital signs and health status, reporting any changes to the RN or physician.
  • Assist with daily living activities, including bathing, dressing, feeding, and mobility.
  • Maintain accurate and timely documentation of the individual's care, observations, and interventions.
  • Collaborate with the health care team to develop, implement, and evaluate individualized care plans.
  • Ensure compliance with all state and federal regulations and company policies.

Qualifications
:

  • Valid and active LPN license in Virginia.
  • Minimum of 1 year of nursing experience preferred, particularly in home health, private duty nursing, or a similar setting.
  • Experience working with individuals with developmental disabilities (DD) is a plus.
  • Current CPR certification.
  • Strong clinical and assessment skills.
  • Excellent communication and interpersonal skills.
  • Ability to work independently and manage time effectively.
  • Compassionate, patient-focused, and reliable.
  • Valid driver's license and reliable transportation.
