As a Licensed Practical Nurse (LPN) working with Intensa Care, you will be responsible for providing personalized, one-on-one care to individuals with developmental disabilities. You will ensure that each patient receives high-quality, person-centered care, and work closely with their family, and our team of healthcare professionals. Each shift, you will be supported by a Certified Nursing Assistant (CNA) or Direct Support Professional (DSP), who will be present in the home to assist you.
Key Responsibilities
:
- Provide direct, 1:1 patient care for individuals with developmental disabilities (DD) under the supervision of a Registered Nurse (RN).
- Administer medications, treatments, and prescribed procedures.
- Monitor and assess patients' vital signs and health status, reporting any changes to the RN or physician.
- Assist with daily living activities, including bathing, dressing, feeding, and mobility.
- Maintain accurate and timely documentation of the individual's care, observations, and interventions.
- Collaborate with the health care team to develop, implement, and evaluate individualized care plans.
- Ensure compliance with all state and federal regulations and company policies.
Qualifications
:
- Valid and active LPN license in Virginia.
- Minimum of 1 year of nursing experience preferred, particularly in home health, private duty nursing, or a similar setting.
- Experience working with individuals with developmental disabilities (DD) is a plus.
- Current CPR certification.
- Strong clinical and assessment skills.
- Excellent communication and interpersonal skills.
- Ability to work independently and manage time effectively.
- Compassionate, patient-focused, and reliable.
- Valid driver's license and reliable transportation.
