Home Therapy RN – Dialysis Position
Responsible for empowering, engaging, and training dialysis patients on peritoneal and home hemodialysis using state‑of‑the‑art training programs and equipment. Works primarily in the clinic setting and conducts initial home visits with another staff member present. Functions as an onsite Patient Educator and Case Manager with flexible shifts.
Principal Duties and Responsibilities- Provide clinically competent care including assessment, planning, intervention, and evaluation for an assigned group of patients.
- Delegate appropriate tasks to direct patient care staff including RNs, LVN/LPNs, and Patient Care Technicians.
- Collect and analyze dialysis data for assigned patients and document adjustments to treatment plans.
- Assess and document patient/family learning needs, providing ongoing education.
- Direct and provide, in collaboration with the patient and care partners, all aspects of safe and effective dialysis delivery.
- Administer medications as prescribed or per approved algorithms, documenting justification and effectiveness.
- Initiate or assist with emergency response measures.
- Serve as a resource, leader, coach, mentor, and role model for new and incumbent employees.
- Ensure correct laboratory collection, processing, and shipping procedures; reschedule missed or insufficient collections.
- Identify expected outcomes and update the nursing assessment and plan of care in collaboration with the interdisciplinary team.
- Ensure patient awareness of transplant and treatment modality options.
- Assist in identification, evaluation, selection, and education of Home Dialysis candidates and Home Partners.
- Assess candidate’s home environment and family readiness for dialysis treatments in the home.
- Train home dialysis patients and/or Home Partners on safe operation, maintenance of equipment, and supplies through a formal standardized Home Dialysis Training Program.
- Participate in education and quality improvement projects at the facility and area level as directed by Supervisor.
- May serve as a preceptor to new employees.
- Complete CAP requirements to maintain or advance.
- Perform all other duties assigned by Supervisor.
Requires heavy lifting, patient movement, standing periods, and the ability to bend over. May need to move machines and equipment up to 200 lbs. and lift chemical and water solutions up to 30 lbs. at up to 5 feet. Work environment is a health care facility with controlled temperature and moderate noise. Exposure to infectious materials may occur. Includes on‑call rotation, night, weekend, holiday, or as defined by program needs.
May require travel to training sites, other facilities, and patient homes.
Provides oversight of RNs, LPNs/LVNs, Patient Care Technicians, and Home Therapies Care Team Assistants as a Team Leader or designated Nurse in Charge after completing all FKC education and training requirements. Must have a minimum of 3 months experience in home dialysis therapies as a RN.
Education and Licensure- Graduate of an accredited School of Nursing.
- Current state licensure.
- Current or successful completion of CPR BLS Certification.
- Must meet practice requirements in the state of employment.
- Minimum 12 months experience as an RN; entry level for RNs with 2+ years Nephrology Nursing experience within the last 2 years.
- Minimum 3 months experience in home dialysis therapies as a RN.
- Compliance with drug‑free workplace policies.
