LTSS Service Coordinator – RN Clinician

The ideal candidate should reside in Colonial Beach, King George, or Westmoreland, VA. This field-based role primarily operates in the field, traveling to client sites as required, with occasional office attendance for meetings or training. Alternate locations may be considered for candidates residing within commuting distance of an office.

• Overall management of members’ cases within the scope of licensure.
• Provide supervision and direction to non-RN clinicians in the member’s case in accordance with applicable state law and contract.
• Develop, monitor, evaluate, and revise the member’s care plan to meet the member’s needs, optimizing health care across the continuum.
• Perform face-to-face clinical assessments for identification, evaluation, coordination, and management of member needs (physical health, behavioral health, social services, and long-term services and supports).
• Identify members for high-risk complications and coordinate care with the health care team.
• Manage members with chronic illnesses, co-morbidities, and/or disabilities to ensure cost-effective and efficient utilization of health benefits.
• Obtain a thorough and accurate member history to develop an individualized care plan.
• Establish short- and long-term goals in collaboration with the member, caregivers, family, natural supports, physicians, etc.
• Assist with implementation of member care plans by facilitating authorizations/referrals for utilization of services within benefit structure or extra-contractual arrangements, as permissible.
• Interface with Medical Directors, Physician Advisors, and Inter-Disciplinary Teams on development of care management treatment plans.
• Assist in problem solving with providers, claims, or service issues.
• Direct and/or supervise work of any LPN/LVN, LSW, LCSW, LMSW, and other licensed professionals, assigning tasks, verifying information, conducting assessments, and reviewing performance.

• Registered Nurse (RN) license in the applicable state(s) – current and unrestricted.
• Minimum 3 years of experience working with individuals with chronic illnesses, co-morbidities, and/or disabilities in a Service Coordinator, Case Management, or similar role, or equivalent education and experience.
• May require state-specified certification based on state law and/or contract.

• MA/MS in Health or Nursing preferred.
• Willingness to travel to work sites and other locations as necessary.
