Provide comprehensive primary care in an outpatient, team-based setting, including routine and acute patient visits, annual wellness exams, health risk assessments, care coordination, patient education, and population health initiatives. Collaborate with physicians, nurses, medical assistants, and specialists to deliver high-quality, patient-centered care.
Qualifications:
- Master's degree in Nursing or Physician Assistant studies
- National certification as an FNP, AGPCNP, ANP, GNP, or PA-C
- Active state NP/PA license and DEA license
- Experience in primary care, family medicine, internal medicine, or geriatrics preferred
- Knowledge of HCC documentation, ICD-10 coding, and Health Risk Assessments (HRAs) is a plus
- Strong teamwork, communication, and patient-centered care skills
