Schedule: Full-Time | Day Shift

Pay: LPN: $36.00–$40.00/hour | RN: $40.00–$43.00/hour

Lakeside Health & Rehabilitation is seeking an experienced Certified Wound Care Nurse (RN or LPN) to join our clinical leadership team. If you have a passion for wound prevention, skin integrity, and improving resident outcomes, this is an opportunity to bring your expertise to a team that values quality care and strong clinical leadership.

In this important role, you will oversee wound prevention, assessment, treatment, documentation, and education while partnering with our interdisciplinary team to promote positive healing outcomes.

• Perform comprehensive wound and skin assessments and assist in developing individualized treatment plans.
• Monitor and manage acute and chronic wounds, pressure injuries, surgical wounds, skin tears, and other complex skin conditions.
• Provide hands‑on wound care treatments utilizing current evidence‑based practices.
• Conduct weekly wound rounds and monitor resident progress.
• Collaborate with physicians, nursing leadership, dietary, therapy, and other interdisciplinary team members to optimize healing outcomes.
• Maintain accurate and timely documentation of assessments, treatments, measurements, and progress.
• Educate nursing staff, residents, and families on wound prevention, treatment, and skin integrity.
• Monitor infection prevention practices related to wound care and skin health.
• Ensure wound care practices remain compliant with state, federal, facility, and regulatory requirements.
• Participate in Quality Assurance and Performance Improvement (QAPI) initiatives related to wound care and skin integrity.

• Current, unrestricted Virginia RN or LPN license required.
• Current Wound Care Certification required — WCC, CWCN, CWON, WOCN, or equivalent.
• Previous wound care experience in a skilled nursing, rehabilitation, long‑term care, or similar healthcare environment strongly preferred.
• Strong clinical assessment and critical‑thinking skills.
• Excellent documentation, organization, and communication skills.
• Knowledge of current wound care treatments, protocols, infection prevention practices, and regulatory requirements.
• Ability to educate, mentor, and collaborate effectively with nursing staff.
• Commitment to providing exceptional, resident‑centered care.

• Licensed Practical Nurse (LPN): $36.00–$40.00 per hour
• Registered Nurse (RN): $40.00–$43.00 per hour
