We are a clinician-led and owned agency serving patients throughout Central Virginia, and we are looking for dedicated Registered Nurses who want to make a meaningful difference in patients’ lives — one home visit at a time.

Home health gives you the opportunity to slow down, connect with your patients, and provide care that truly matters. Instead of rushing from room to room, you get to see the full picture: the patient, their family, their home environment, their goals, and the barriers standing in the way of recovery.

Our RNs provide skilled nursing visits, patient assessments, care coordination, medication education, wound care, disease management, post-hospital follow-up, and support for patients recovering from illness, surgery, injury, or a change in condition.

• Skilled nursing visits in the patient’s home
• Start of care, recertification, resumption of care, and discharge assessments
• OASIS documentation and care planning
• Medication reconciliation and patient education
• Wound care and chronic disease management
• Communication with physicians, families, therapists, and the care team
• Vital sign monitoring
• IV Infusions
• Support for chronic disease management, including advanced heart failure, transplants, etc
• Documentation of patient progress and changes in condition
• Helping patients remain safe, independent, and supported at home

• Active Virginia RN license
• Current CPR certification
• Reliable transportation
• Valid driver’s license and auto insurance
• Home health experience preferred
• OASIS experience preferred for RN applicants
