Supportive​/Outpatient Psychiatry RN

SUMMARY OF POSITION

The Psychiatric-Mental Health RN (PMH-RN) in Supportive Housing Services is a member of the interdisciplinary team which partners with individuals to achieve independence in their physical and mental health care needs. Provide health promotion, screening, maintenance, and education based on the individual’s integrated health care needs. Assessment and monitoring of health care, case coordination, and self-care needs are part of the ongoing nursing process.

Administration and oversight of treatment regimens as ordered by the prescribing provider. Support of non-clinical staff related to patient’s medical/clinical needs. Management of all patient/client care needs related to medication, point of care lab, and infection control. The PMH-RN in Outpatient Psychiatry prepares patients for the clinic appointment with the psychiatric physician or nurse practitioner, obtaining history and vital signs, ensuring ordered labs/procedure results are received, providing psychoeducation, and administration of prescribed injectable medications.

This RN will also ensure timely telephone triage responses, lab result notifications, and medication prior authorization processes.

• Assist in facilitating linkage to health care services
• Assist in discharge planning, as appropriate
• Admission nursing assessments upon move-in to the group home, including General head-to-toe physical assessment
• Medication Review Allergy Review Verify TB test results
• Ensure any ordered homecare equipment is present and working properly (CPAP, O2, Ambulation Aides)
• Ensure Primary Care and Psychiatric Care engagement
• Medications:
  Medication reconciliation and availability for self-administration
• Medication Administration Record (MAR) management
• Oversight and management of controlled medications
• Administration of medications on site, as applicable
• Medication disposal
• Labs & Waived CLIA Testing:
  Perform approved Point of Care (POC) testing, as ordered
• Monitoring and reporting of lab results
• Infection & Exposure Control:
  Support staff with implementation of Infection & Exposure Control policies and procedures
• Provide education and support to staff and patients/patient/clients as needed:
  Regulatory and compliance documentation and record maintenance

• Meets dress code standards, appearance is neat and clean
• Completes annual education requirements
• Maintains regulatory requirements
• Maintains and ensures patient confidentiality at all times
• Reports to work on time and as scheduled; complete work within designated time
• Represents the organization in a positive and professional manner in the community
• Wears identification while on duty

Education Level: High School diploma or equivalent. Graduation from accredited RN program.
