Hybrid RN Case Manager: Infusion & Telehealth

Ponos Care is a physician-led, value-based healthcare organization delivering holistic care for patients with complex, chronic, inflammatory and immune-related conditions. This RN Case Manager role blends telephonic case management with travel–infusion duties to coordinate care across multiple states and services.

You will conduct comprehensive assessments, create personalized care plans, and collaborate with interdisciplinary teams to support independence and quality of life for Medicaid
