The bilingual caseworker supports individuals and families facing social, economic, health, or behavioral challenges by providing guidance, advocacy, case management and access to community resources and services. The role involves assessing client needs, developing individualized service plans, and coordinating care and referrals, and supporting clients in achieving stability and self-sufficiency while upholding the mission and values of Sacred Heart Center.

Key Responsibilities
  • Conduct client assessments to identify needs, strengths, and goals
  • Develop and implement individualized service or treatment plans
  • Connect clients with resources such as housing, healthcare, employment, or counseling services
  • Monitor client progress and adjust service plans as needed
  • Maintain accurate, timely, and confidential case notes, documentation, and monthly reports
  • Provide crisis intervention, emotional support, and appropriate referrals when necessary
  • Collaborate with internal staff, partner organizations, and multidisciplinary teams to ensure coordinated client support
  • Participate in outreach activities, workshops, and community events as assigned
  • Ensure compliance with organizational policies, grant requirements, confidentiality standards, and relevant regulations
  • Uphold and represent the mission, values, and community-centered approach of Sacred Heart Center
  • Other Duties as Assigned.
Qualifications
  • Bachelor’s degree in social work, psychology, human services, or a related field
  • Minimum of one year of experience in case management, social services, nonprofit work, or a related field preferred
  • Strong communication and interpersonal skills
  • Fluency in Spanish and English, both written and verbal, required
  • Ability to handle sensitive and confidential situations with professionalism, empathy, and cultural sensitivity
  • Strong organizational, time management, and documentation skills
  • Knowledge of community resources, immigrant services, and social service systems
  • Ability to work independently and collaboratively in a fast-paced environment
Preferred Skills
  • Experience working with immigrant and culturally diverse communities is preferred.
  • Knowledge of trauma-informed and culturally responsive practices
  • Crisis intervention and de-escalation skills
  • Strong problem-solving and critical-thinking abilities
  • Experience with case management software and data tracking systems
  • Ability to build trust and maintain positive relationships with clients and community partners
  • Fast paced work in office, community center
  • Regular participation in outreach events, public facing programs
  • May involve working with high-risk or vulnerable populations from a variety of socioeconomic backgrounds and cultural backgrounds.
  • Some evening and weekend hours may be required based on program needs and community events.
Physical Demands
  • The ability to stand, walk, and sit for extended periods of time, as the role may involve attending events, meetings, or conferences.
  • The capacity to move around the office, event venues, and other locations as required, including the ability to climb stairs, navigate uneven terrain, and carry out physical tasks associated with event setup and teardown.
  • Proficient use of hands and fingers to handle paperwork, operate office equipment and perform tasks such as filing, typing, and data entry accurately and efficiently.
  • Candidate must have reliable transportation and the ability to travel to various locations throughout the service areas as needed to fulfill job responsibilities.
  • The ability to lift and carry event materials, supplies, and promotional items, which may vary in weight but could range from light to moderately heavy objects, 50 pounds.
FLSA Status

Non-Exempt- 40 hours a week

Hourly Rate

Starting at $21.00 per hour, commensurate with experience and qualifications.
