This position is responsible for providing comprehensive, structured case management services for emergency shelter residents. Learns and adapts to the uniqueness of a resident’s history to determine the most effective relationship approach. Develops comprehensive program plans/goals and evaluates residents’ progress through one‑on‑one sessions and diagnostic tools. Advocates for community resources and services that will enable residents to resolve or ameliorate the underlying causes of their homelessness.
Key Responsibilities- Conduct resident intake and initial needs assessment at the time of entry to the shelter.
- Provide linens, toiletries, and hygiene products; replenish as needed throughout residents’ stay.
- Meet individually with residents to discuss and evaluate progress, feelings, impressions, changes, and personal development; prepare accurate and up‑to‑date records documenting the same.
- Prepare and maintain case records and logs; ensure accuracy and completeness; enter pertinent information into the Homeward Community Information System (HCIS).
- Refer residents to additional counseling resources if needed and maintain awareness of progress with external case management professionals, working as a team to meet the holistic needs of the individual.
- Associate degree in social work, human services, or related field from an accredited college or university; AND one year experience providing direct case management social services including accessing residents’ needs and developing individual, comprehensive, long‑term action plans for recovery utilizing a wide variety of resources.
- Any equivalent combination of training and experience providing the required knowledge, skills, and abilities.
- Work is performed in a shelter facility with little or no physical discomfort associated with changes in weather or discomforts associated with noise, dust, dirt, and the like.
- Ability to meet attendance requirements.
- Ability to read, write, and communicate in English.
- Ability to communicate clearly and effectively both orally and in writing.
- Ability to work on multiple cases effectively and efficiently without becoming frustrated or disorganized.
- Limited physical effort required: walking, standing, lifting and carrying light objects (less than 25 lbs.).
- Duties are usually performed seated, may be relieved by brief or occasional periods of standing or walking.
