You will complete comprehensive assessments, develop care plans for patients with dementia, and coordinate with caregivers and the patient’s care team across the continuum in the community and clinics.
You will provide dementia training, data collection, outreach, and presentations, plus screen, triage, referral care, and monitor participants.
