The Family Outreach & Housing Navigator provides housing-focused case management, crisis intervention, and resource coordination for families experiencing homelessness or at risk of homelessness. The position serves as the primary case manager for families participating in the City's Family Hoteling Program while providing year-round support to families seeking assistance through the Office of Homeless Services.
Responsibilities include conducting assessments, determining eligibility for emergency housing interventions, developing housing stabilization plans, coordinating referrals, and connecting families to community resources. During the Family Hoteling Program season, the position participates in an after-hours on-call rotation to facilitate emergency hotel placements and provide crisis response.
Duties include but are not limited to- Assess families experiencing homelessness or housing instability to determine eligibility for services and emergency housing interventions.
- Provide housing-focused case management, crisis intervention, and individualized service planning.
- Assist families in accessing shelter, permanent housing, public benefits, employment, childcare, healthcare, and other community resources.
- Serve as the primary case manager for families participating in the Family Hoteling Program, including coordinating placements, monitoring progress, and facilitating successful exits from the program.
- Develop and maintain collaborative relationships with community partners to ensure coordinated service delivery.
- Maintain accurate client records, case notes, and data in HMIS and other required systems.
- Participate in coordinated case conferencing and represent the Office of Homeless Services at community meetings as assigned.
- Participate in an after-hours on-call rotation during the Family Hoteling Program season to assess referrals and coordinate emergency hotel placements.
- Perform other related duties as assigned.
- Bachelor's degree in social work, Human Services, Psychology, Sociology, Public Administration, or a related field.
- Two years of experience providing case management, housing navigation, or related human services; or an equivalent combination of education and experience.
- A valid driver's license is required.
- Master's in social work.
- Experience working with families experiencing homelessness.
- HMIS proficiency, and knowledge of Coordinated Entry, and Housing First practices.
- Knowledge of homeless services, housing resources, and case management practices.
- Ability to assess client needs, develop service plans, and coordinate community resources.
- Strong communication, organizational, and problem-solving skills.
- Ability to manage multiple cases while exercising sound professional judgment.
- Proficiency with Microsoft Office and HMIS or similar case management software.
ENVIRONMENTAL HAZARDS:
Working conditions may include exposure to travel to other locations within the City of Richmond as well as outside of the City of Richmond; exposure to hazardous physical conditions such as mechanical parts, electrical currents, vibrations, etc.; atmospheric conditions such as fumes, odors, dusts, gases, and poor ventilation; inadequate lighting; intense noise; and environmental hazards such as disruptive people, imminent danger, and a threatening environment.
PHYSICAL REQUIREMENTS AND WORKING ENVIRONMENT:
